Message from Bob Duval:

HSC Information Technology Services has requested that we provide them information on all Projects that involve data storage of any sort, with some information on the data used/required for the activity. This is related to, but not the same as, the request I sent earlier on software and data usage.

The HSC IT form attached requests that you to identify “projects” and the data required. If you could please list your projects along with any approved IRB protocol #, or any project containing active storage of sensitive/controlled data that would be great. If the data is from an earlier project, but is still be retained it also should be listed if it’s sensitive as well.

For classifying data here is a guidance from our (HSC) policy. If it is all PHI then it would be restricted and most likely severe impact on confidentiality and integrity levels, availability level could vary on the answer :

If you will all complete the IT form attached, and return it to myself, Jessica and Sarah, that would be very helpful. If you have no projects that apply, please fill out one line with your name as PI and use “NA” in the other boxes. We will compile the forms for IT from what you send back. Please return by August 24^th.
